Items included in the rebate are CoverGirl, Pantene, Secret, Venus, Ivory, Herbal Essence, Aussie, Clairol, Safeguard, Olay, Crest 3D Whitestrips, Always, Tampax, Head & Shoulders, or Vidal Sassoon Products purchased between 10/15 and 12/31.
To score the rebate, mail the original form, write in the UPC of the items on the form, and include the original sales dated receipts with purchases circled.
The rebate applies to the price paid after any coupon or discount has been applied.
