Spend $50 on Olay Facial SkinCare between July 7th and September 6th, 2013 – complete the form HERE for a $20 pre-paid card by mail.
You will need this completed form, the original dated sales receipt(s) and the UPC Codes from the products written in the spaces on the offer form.
The $50 threshold must be after coupons and all discounts – those of you who shop at CVS, most often your ECBs will also be taken into consideration.
